These Terms of Service (hereinafter referred to as "the Terms") set forth the conditions of use for all medical services (hereinafter referred to as "the Services") provided by UTOPIART CLINIC EBISU (hereinafter referred to as "the Clinic"). All individuals using the Services (hereinafter referred to as "the Patient") shall do so in agreement with these Terms.
Article 1 (Scope of Application) These Terms shall apply to all Patients using the Services provided by the Clinic.
Article 2 (Medical Policy)

The Clinic aims to provide high-quality cosmetic medicine tailored to each individual Patient.

Prior to any treatment, the Clinic will provide a thorough explanation of the effects, risks, side effects, and costs, and will proceed with the treatment only after obtaining the Patient's informed consent.

The Clinic respects the privacy of the Patient and is committed to the protection of personal information.
Article 3 (Appointments)

The Clinic operates on a strict appointment-only basis. Those wishing to use the Services should make an appointment through the methods specified by the Clinic.

Changes and cancellations of appointments shall be subject to the separately established "Cancellation Policy."
Article 4 (Refusal or Discontinuation of Treatment) The Clinic may refuse or discontinue treatment if the Patient falls under any of the following categories:

Engages in violence, abusive language, intimidation, or other disruptive behavior towards other patients or Clinic staff.

Is under the influence of alcohol or in a condition that the Clinic deems unsafe for providing medical treatment.

Provides false information on medical history forms or other documents.

Fails to follow the Clinic's instructions or is uncooperative with the treatment.

In any other case where the Clinic deems it inappropriate to provide treatment.
Article 5 (Handling of Personal Information) Personal information of the Patient acquired by the Clinic in the course of providing the Services shall be handled appropriately in accordance with the Clinic's separately established "Privacy Policy."
Article 6 (Fees and Payment)

The Services provided by the Clinic are private medical treatments and are not covered by public health insurance.

Fees are based on the price list posted on the Clinic's website or displayed within the Clinic.

Payment is due on the day of treatment, either in cash or by a credit card accepted by the Clinic.
Article 7 (Prohibited Acts) The Patient shall not engage in the following acts when using the Services:

Acts that violate laws, regulations, or public order and morals.

Photography, audio recording, or video recording within the Clinic without permission.

Smoking within the Clinic premises.

Acts that infringe upon the privacy of other patients.

Acts that may interfere with the operation of the Clinic.
Article 8 (Changes to the Terms) The Clinic may, when deemed necessary, amend these Terms at any time without prior notice to the Patient. The amended Terms shall become effective from the time they are posted on the Clinic's website.
